A 6-year-old boy with a medium-sized congenital divided nevus of the right eyelid was referred to our department. Purpose: To report the clinical, pathological, and immunohistochemical features of the first pigmented spindle cell nevus (PSCN) of Reed documented to have appeared in the eyelid. Methods: The findings of clinical and histopathological examination are presented, along with differential diagnoses and a review of the pertinent literature. Melanoacanthoma is very rare variant of seborrheic keratosis presenting as a rare benign pigmented lesion composed of both melanocytes and keratinocytes usually presenting over the head, neck, and trunk of elderly people. All eyelid tumors should be photographed for comparison with future examinations. if possible, patients should keep a copy of their baseline photographs. Malignant melanoma of the eyelid is distinguished from an eyelid nevus in that it can be variably pigmented, change color, bleed and/or grow.
One should consider melanoacanthoma for a brownish black nodular lesion of the eyelid, and histopathological examination is mandatory for the confirmation of diagnosis and ruling out of melanoma or nevus.
